Primary health care is the first point of contact a person has with the health system. Our inter-professional team of physicians, nurse practitioners, nurses, pharmacist, social worker, dietitian and others collaborate to provide services and programs to our clients. Working together, this team responds to the factors that impact the health and well-being of our clients. They ensure that our clients receive the best possible care. Primary care includes the prevention, diagnosis, treatment and follow-up of various health conditions, including referrals to specialists and diagnostic services.

What to Expect
When you come in for your first appointment we will review your completed forms including signing consent to release your previous medical records, discuss our protocols and introduce you to your assigned physician. We ask that you bring in all medications including over-the-counter that you are taking to be reviewed. Your information is kept in an electronic medical record.
Sometimes, when you are sick and your assigned provider is not available, you may be offered an appointment with another provider. This may be a nurse practitioner or doctor.
