Gateway Community Health Centre

Primary Care

Our services are focused on enabling individuals to increase control over, and to improve, their health.

Primary Care

Primary health care is the first point of contact a person has with the health care system when they have a health problem or issue that is not an emergency.

Our interprofessional team of physicians, nurse practitioners, nurses, pharmacist, social worker, dietitian and others work together to provide services and programs that meet the specific needs of our clients and community including the diagnosis and treatment of health problems; promotion of good health; prevention of health problems; early intervention; and the management and care of ongoing and long-term health conditions.

Primary care services include –

      • Scheduled  medical appointments
      • Comprehensive assessment to determine resources to meet your needs
      • Referrals and follow up of community resources
      • Medication review appointments and follow-up
      • Lab services
      • Home visits
      • On call 24/7
      • Same-day urgent care appointments
      • Cancer screening
      • Follow-up after an emergency department visit or hospital discharge

Ontario Telemedicine Network (OTN)

Telemedicine uses video cameras and TV screens to connect clients to providers, specialists, or other health care professionals throughout Ontario reducing the need to travel to a big city to receive care. There is no cost to clients using Telemedicine services. Telemedicine nurses from Belleville Quinte West CHC coordinate on site appointments at Gateway CHC and assist the specialist and clients with required assessments before and during their appointments.

If you have questions about Telemedicine and referrals, you can call the Telemedicine Team at the Belleville Quinte West CHC.
