Consent for Use of Email Address
By providing my consent and email address, I confirm and acknowledge that:
- Only the email address I provide will be used by Gateway for communication to me for appointment reminders, information about programs and services, and document exchange.
- I understand that this service is not to be used for communicating emergency or urgent needs or information, and that Gateway will not be monitoring the inbox 24/7 nor responding immediately.
- I understand and agree that any information provided by me through documents and via email may form part of my health record.
- I understand that Gateway follows strict rules and uses all available security measures to protect my privacy, however, I acknowledge that Gateway cannot guarantee the security of email exchanges to and from the Health Centre.
- It is my responsibility to update Gateway about any email address change.
- I may withdraw my consent at any time for Gateway to use my email, in writing.
- I understand that if I am signing on behalf of a minor child, this consent will be null and void when the child reaches age 16 at which time the child will have the option of signing their own consent to use the Pomelo Health virtual portal.
